The Advanced Practice Nurse provides services to the acute and chronically out-patient diabetes population. The APN collaborates with physicians to prevent 30-day readmissions while integrating education, research, management, leadership, and consultation into the clinical role.

* Serves as a resource for physicians to facilitate the admission and coordination of care for the acute and chronically ill patient diabetes population. Physician collaboration includes but is not limited to providing the patient History and Physicial, ordering and evaluating of diagnostic tests and procedures, developing the plan of care, discharge planning and follow-up with Attending physician post discharge. Identifies patient deviations from normal, co-manages selected patients and makes referrals as appropriate to ensure patient have required suppport to manage acute and chronic illness post discharge. Completes all required documentation in a timely manner including the EMR, CPOE, dictation and narrative. Complies with established clinical practice guidelines and in accordance with the privileges granted by the Department of Family Medicine.

* Provides individualized patient/family health counseling, referrals and health advocacy services to acute and chronically ill out-patient diabetes population, their families/significant others to prevent all cause readmissions and assist with the clinical mangement of CMS Core Measures including but not limited to diabetes, chronic renal failure, heart failure, COPD and pneumonia. Contributes to Multidiscliplinary patient care meeting.

* In conjunction with the Continuing Care staff , initiates patient/ family meetings and to determine post discharge needs. Contacts patients post discharge to ensure that com

* Collaborates with Patient Care Leadership/Diabetes Liasion/patient care staff to improve the quality of care and assist in developing medical management expertise within our nursing staff.

* Serves as a resource for the staff nurse to facilitate the coordination of care and the movement of patients through the continuum.

General Job Requirements

* Illinois License for Nurse Practioner in Family or Acute Care

* Clinical abilities as demonstrated through training, education, certification and experience

* Strong interpersonal communication and organizational skills

* Strong leadership skills

* Strong analytic/statistical skills

* Computer/software skills

* Bilingual (English/Spanish) preferred
