Case Manager A-2 Location: Lebanon, OH Job Id: 1256 # of Openings: 1 Talbert House has a career opening for a Case Manager A-2 WORK LOCATION : 5234 St. Rt. 63 NORMAL WORK HOURS : 1st Shift FLSA STATUS : Hourly Non-Exempt JOB TYPE : Full Time JOB CODE : 0000 - 305 POSITION SUMMARY The Case Manager will provide excellent customer service and maintain a caseload of clients to assist them to more effectively function in the community or to prepare them for reintegration into the community. Duties and additional experience or requirements may vary by position. ESSENTIAL JOB FUNCTIONS: Maintain caseloads and productivity by ensuring regular contact with clients. Monitor and assess progress per agency policy, regulatory requirements and best practices. Meet with clients individually or in a group to assess needs, collaborate with clients, families and other case managers andsite staff. May provide but is not limited to: crisis intervention, advocacy, medication monitoring, transportation assistance, financial assistance, referral and linkage to community resources. May transport clients. Prepare clinical reports and individualized service plans, utilizing agency electronic clinical documentation software. Assure documentation is in compliance with contract, regulatory and agency requirements and best clinical practices. Efficiently operates personal computer to enter, edit and/ or retrieve data and to produce reports and documentation. Effectively works as a team member including participating in all required trainings, ensure the safety of others and participate in Peer Review/CQI process. QUALIFICATIONS REQUIRED: Bachelors degree in Behavioral Science or related area or Equivalent combination of experience and training A valid drivers license and current auto insurance may be required. JOB SPECIFIC REQUIREMENTS: Required to work one eveningper week and anoccasional Saturday. To perform this job successfully, as individual must be able to perform each essential duty satisfactorily.
