Job Description

JEVS Human Services has an exciting opportunity within our Nursing Home Transition program as a Community Transition Specialist. The Nursing Home Transition Program provide financial support, service coordination, and acts as an advocate for individuals who desire to make the transition from living in a nursing home to living on their own in the community.

As the Community Transition Specialist you will provide case management and support to individuals in the Nursing Home Transition Program. You will be responsible for engaging in daily, billable case management activities to help individuals relocate from nursing homes back to the community.

Day to Day:

* Travel to local nursing homes; conduct intake and assessment interviews, identify community needs, locate housing, connect Participants to resources, etc.

* Collaborate with nursing home social work staff, providers and vendors, family, and Service Coordination to facilitate a successful transition

Skills/Requirements

Requirements:

* Bachelor's degree in sociology, social welfare, psychology, gerontology or another behavioral science required

* 3+ years' experience in a related field required

* Must be computer proficient; experience with databases and Microsoft Office Suite required

* Communication and customer service skills are essential

* Must have valid Driver's license and personal vehicle
