* Identifies and locates housing options suitable for homeless transition age youth (TAY).

* Develops support systems to meet individual's needs; coordinates and facilitates move-in activities, rental negotiations, and service referrals as needed

* Creates and maintains a housing database linking tenants to apartments

* Communicates with landlords and familiarize them with the Cal-OES program and the unique needs of homeless youth

* Facilitates the use of community resources to assist both tenants and landlords in the event of problems leading to potential eviction

* Coordinates follow-up services with case management team

* Conducts habitability and lead-based paint inspections of housing options as needed

* As a member of the program case management team, provides information referrals, crisis intervention, and assistance with permanent housing placement for participants

* Assists outreach teams in providing outreach and engagement services to literally homeless youth

* Assists case managers in conducting the VI-SPSDAT and CSE-IT comprehensive screenings and assessments with youth to collect functional, environmental, financial, employment, housing, educational, and health information, as appropriate, to develop an Individual Service Plan

* Additional duties as assigned



* Bachelor's degree in social services or 2 years of housing/ case management experience


* Candidate should possess 1 to 2 years of housing/ case management experience

* Two year experience providing trauma-informed care to homeless or other vulnerable youth

Skills Required to Perform this Position:

* Ability to develop relationships with homeless youth, their families, and landlords, as well as a variety of service stakeholders (schools, health and public benefit agencies)

* Experience and knowledge of treatment plan design and case management

* Effective written and oral communication skills

* Must be reliable and have the ability to multi-task

* Must be able to communicate effectively and to write clearly to be able to complete required paperwork

* Must be able to perform basic arithmetic (to count)

* Must be able to work effectively with people from all walks of life with compassion, to be covered by VOA's insurance carrier

Preferred Skills for this Position:

* Bilingual (Spanish) (Preferred)
