Provides outpatient nurse care for our patients and assisting physicians in all clinical areas of the practice. Duties will include determining who had contact with diagnosed patients, conducting remote assessments of symptoms, and ensuring that those placed under quarantine comply with isolation regulations. This includes providing case investigation and contact tracing management to patients assigned to Student Health and Occupational Health. Assist department management in coordination of patient care and clinic operations to include, but not limited to: being the primary Super User directly responsible for assisting providers and clinic staff with immediate EHR concerns, assists providers and clinic staff with outstanding tasks for patient centered quality care, and is a line of contact between clinic staff and department management in giving detailed clinical operations reports regarding clinic staff and provider’s concerns to implement quality improvement measures. Occasional night and weekend hours as needed.
+ Coordinating contact tracing efforts with Supervisors and Case Investigators from the Local Health Department.
+ Conducting interviews in a professional manner that reflects emotional and cultural awareness.
+ Collecting and recording accurate information using Customer Relationship Management (CRM) software.
+ Calling newly diagnosed patients to trace individuals they had contact with and determining exposure.
+ Interviewing traced contacts, assessing symptoms, and referring them for testing.
+ Providing contacts with regulated information on isolation and quarantine procedures.
+ Collaborating with designated local authorities in supporting the isolation and quarantine of individuals.
+ Maintaining ongoing virtual communication to monitor symptoms.
+ Facilitating health checks and test, as well as referring contacts to available community resources.
+ Complying with all Health Department regulations, including those aimed at protecting personal information.
+ Case Investigation – Interviewing clients with COVID-19, eliciting their close contacts, monitoring the clients for COVID-19 symptoms, connecting clients to resources to support self-isolation
+ Contact Tracing – Notifying close contacts of their potential exposure, referring them to testing, monitoring them for COVID-19 symptoms, connecting contacts to resources to support self-quarantine
+ Completes all tracking and reporting requirements for outcomes and evaluation.
+ Maintains appropriate professional standards including confidentiality and provides appropriate follow-up for patients.
+ Performs other duties as assigned.
EVMS Drive Thru Clinic Responsibilities:
+ Manage multi-specialty clinical services delivered at drive up site
+ Administer immunizations, testing (i.e. Flu, COVID, etc.)
Completion of a program of instruction for licensure as a Practical Nurse and possession of a current Virginia license. One (1) year of experience as an LPN. Completion of BLS class and re-certification every 2 years. Possess basic knowledge of educational behavioral approaches to change behavior.
