The Homecare Social Worker provides emotional support to individuals and families in all phases of illness. The Home Care Social Worker will provide short term interventions to patients to remove clear and direct impediments to effective treatment of the patients medical condition or to patients rate of recover. This person works collaborates with an interdisciplinary team in order to maximize clinical outcomes. The social worker may act as a preceptor to new social work staff.
This position features a $6000 sign on bonus
Provides appropriate information/education to patients/caregivers including patient/caregiver rights and responsibilities.
Addresses psychosocial needs in a holistic manner in collaboration with the patient, caregiver, and IDT.
Has knowledge of community mental health system and services
Is an engaged and collegial member of the IDT.
Has knowledge of effective crisis intervention
Regularly communicates and collaborates with outside agencies to ensure the coordination of needed services for patient and families
Provides assistance for and coordinates long term facility placement for patients
Particpates in discharge planning process and in the provision of smooth, well transitioned care.
Provides short term counseling and support related to illness, adjustment or grief
The ideal candidate will have:
Licensed at the Master level by the State Board of Social Work licensing required.
Maine Driver's License required
Master's degree in Social Work required
1 year or more of pertinent experience preferred
