Supplemental Medication Reconciliation Technician: will work on Meridian Park Transitions of Care team, focusing on Medication Reconciliation in the Emergency Department and on inpatient units. Involves obtaining best possible home medication list by talking to patient, family and using online resources. Work closely with ED and TOC pharmacists. Supplemental role to fill in with vacation and sick call coverage, leaves of absence, etc.


Hours of work: 1230-2100.


Future cross training may include other TOC roles: Meds2Beds, Apothecary/retail technician.


Supplemental staff help fill vacations, sick calls and other staffing needs; there are no guaranteed hours with a supplemental position. Possible shifts needed include weekends, Holidays and evenings.


A 3-4 week training period will be needed before working independently. To accommodate training and supplemental shifts, the ideal candidate would not be working full time at another position.


The Pharmacy Transitions of Care Technician, under the direct supervision of a pharmacist, will assist the care team through various phases of the patient’s care including but not limited to: 1) Gathering the best possible medication history that is then utilized by the care team for medication reconciliation, and 2) Facilitates the provision of medications at the time of discharge from the hospital.


Education: High school diploma or equivalent required. Graduate of a pharmacy technician program preferred.


Experience: Minimum 2 years of experience as a pharmacy technician required; retail and/or inpatient hospital experience preferred. Experience applying customer service behaviors and communication skills required. Basic familiarity with computers and keyboard skills and ability to navigate electronic systems applicable to job functions.
