JOB TITLE: Psychiatric Nurse Practitioner/Physician Assistant

DEPARTMENT: Behavioral Health

REPORTS TO: Chief of Behavioral Health

FLSA TBD by HR): Exempt

JOB PURPOSE: Responsible for providing behavioral health care to patients and family in collaboration with primary care physicians, physician assistants and nurse practitioner colleagues throughout the Jordan Health Care setting.

Provides comprehensive behavioral health evaluations, consultations with providers, assists with coordination of care either within the Jordan Health Care setting or referrals to community mental health centers. Performs in short term, episodic model, under the regulations of the Department of Health. Supervised and evaluated yearly by Department Chief and Medical Director. Specialty care and administrative duties as required.

MAJOR RESPONSIBILITIES (Essential functions):

1. Obtains psychosocial and behavioral health history on patients upon family medicine referral, throughout lifespan.

2. Completes physical and mental health assessment according to need and established clinical guidelines

3. Provides counseling specific to developmental needs including symptom management, medication compliance, substance use, risk reduction, family planning, school/work issues and family/relationship issues.

4. Manages common acute behavioral health illnesses, chronic behavioral illness and health maintenance needs, using of clinical laboratory and diagnostic tests/imaging and procedures.

5. Helps to advocate for patients needs within the health care system and with community agencies

6. Refers to specialty services as appropriate, including makes community mental health referrals when long term care is needed.

7. Participate in ongoing Quality Improvement and research projects

8. Assists Family Medicine and Behavioral Health staff in planning future activities of the Department, implementing and assessing Department activities

9. Assists in orientation and precepts graduate nursing students, residents, medical students and new staff as needed , when applicable .

10. Effectively uses technology and resources to accomplish job

11. Complies with NYS guidelines regarding licensing, prescriptions, record keeping

12. Attends department meetings

2. The working condition of the position is: (for example, outside, a typical office setting, etc).

The majority of the working station will be in an office. The evaluations and care of the patient will occur within the office of the nurse practitioner. There is a fair amount of walking back and forth between the office and the nurses' station to collaborate care with the nurse working with the nurse practitioner.

4. EDUCATION AND EXPERIENCE REQUIRED: Masters Degree in Family Psychiatric Nurse Practitioner, Physician Assistant in Psychiatric Mental Health One year of clinical experience working within the field of substance abuse and/or behavioral health. Preferred candidates will have experience working with chronic mental illness. Experience working with multicultural backgrounds.

LICENSES AND CERTIFICATIONS: NYS RN, NPP, PA licensure; National Certification

SPECIAL SKILLS. KNOWLEDGE REQUIRED: Bilingual preferred.
