Job Description

This position implements Individualized Treatment Plans, monitors and observes patient conditions, maintains patient records, maintains the safety of the personnel and patients. This position will assist with maintaining a clean and safe environment, patient treatment planning, and orientation of new patients and staff. Ongoing training/education related to patient care will be provided. Employees will become Certified Nursing Assistants (CNA) through the facilitys training program.

Special Notes: Due to the nature of these positions, all applicants who may be appointed will be required to allow the Department of Health Services to conduct a background check to determine whether the circumstances of any conviction may be related to the job being filled.

Individuals must be able to lift fifty pounds several times a day. In addition, individuals need to have the ability to perform complex motor skills and use specialized equipment. Prior to beginning employment, candidates selected must pass a physical abilities test and pre-employment health assessments, at the employers expense.

Qualifications:

Minimum Requirements:

Candidate must be able to maintain a certified nursing assistant (CNA) license with the State of Wisconsin. Paid training will be provided to those who do not hold their license.

Applicants holding an active CNA licensure from another state that is in good standing may be able to transfer CNA certification to Wisconsin via reciprocity. Access the application by going to the following website: Wi_Nurse_Aid_Program. This must be completed by the start of employment.
