Schedule: Thursday 5p-1a, Friday 5p-8a, Saturday 5p-8a, Sunday 5p-8a

POSITION SUMMARY:

Provides direct patient care using the nursing skills and processes in accordance with applicable scope and standards of practice and with the policies, values and mission of the organization. The RN is a member of the Interdisciplinary Team/Group (IDT/IDG). 

KEY REPORTING RELATIONSHIPS:

Reports to the Clinical Manager or Director of Clinical Services

ESSENTIAL DUTIES:

  • Provides skilled nursing coordination of care to patients (neonates to geriatrics) as determined by the Plan of Care (POC). Uses education, mentoring, guidance and interventions to achieve goals of care.
  • Collaborates as needed across disciplines to coordinate patient care. Participates in the POC in IDT/IDG meetings as assigned.
  • Demonstrates appropriate nursing care including communications with attending provider for ordering of medications and supplies.
  • Provides accurate and timely documentation in Electronic Medical Record (EMR). Uses, clear concise, and professional written and verbal communication.
  • Educates patient/family and community regarding hospice philosophy and palliative treatments.
  • Supervises Certified Nurse Aides (CNA); delegates specific tasks per agency policy.
  • Participates in related services such as community education, team training, staff/board committees and orientation of employees. 
  • Other duties and responsibilities as assigned.
  • If assigned to Inpatient Unit/critical care unit:
  • Meets all the essential duties assigned to RNs
  • Understands acute care needs of patients admitted to the Inpatient/Critical Care Units.
  • Completes initial nursing assessments and admissions processes for newly admitted patients.
  • Manages customer services needs that are unique to a care unit

if assigned to outpatient:

  • Meets all the essential duties assigned to RNs
  • Adapts care to meet needs of patients at their place of residence to include but not limited to the patients’ home, long term care or assisted living facilities. 
  • Completes initial nursing assessments and provides ongoing comprehensive nursing care through medications, treatments, interventions and evaluation as needed.
  • Develops and maintains professional business relationships with facilities by providing excellent customer service to the patient as well as the facility staff. 

If assigned to Triage:

  • Meets all the essential duties assigned to RNs
  • Assesses symptom care needs by utilizing critical thinking and organizational processes. Educates patients and/or care givers as appropriate. Deploys clinical staff to make patient visits as appropriate. 
  • Uses professional and customer service-based demeanor while communicating by telephone, text or email.

  • Performs job duties within the standards of practice framework of the ANA (American Nurses Association).
  • Utilizes critical thinking skills to assess patient and family/care giver needs. 
  • Understands the correlation between excellent patient care and the results of Hospice Item Set, CAHPS, and other publicly-reported industry and/or accreditation quality benchmark programs.
  • Maintains confidentiality of all patients/families. Maintains and respects professional and personal boundaries.
  • Develops and organizes work plan in a high volume environment with rapidly changing priorities.
  • Responsible for meeting case load requirements.
  • Maintains professional and technical knowledge by attending educational workshops, nursing meetings and in-services on a regular basis.
  • Possesses knowledge of necessary computer systems including Microsoft Office, Outlook and agency electronic medical records (EMR). 
  • Utilizes critical thinking skills to problem solve. Promotes a holistic approach to patient care.

POSITION QUALIFICATIONS:

Minimum Education: Graduate of an accredited school of nursing; BSN preferred.

Minimum Experience: 1-2 years of experience in hospice/palliative care, medical/surgical, public health, oncology, acute care or home health.

Required License: Current Colorado RN license. CPR certification. Valid Colorado state driver license and proof of auto insurance.
