Community Living Services (CLS) is an innovative and revolutionary healthcare model that allows seniors to live at home longer by transitioning them out of nursing homes and back into the community by partnering with health plans and Medi-Cal. This inventive, nationally recognized model is one of the most promising for seniors to date, transforming their lives by providing them with a higher quality and more independent life and saving millions of dollars for healthcare systems. CLS is seeking a highly skilled and experienced Regional Director-Inland Empire for its Ontario, CA office. The primary responsibility of this critical position is oversight of all programs within Riverside and San Bernardino counties including programs undergoing implementation and newly-identified expansion programs in the region. The Regional Director will supervise Program Directors, Clinical and Administrative Staff. Other key duties and responsibilities include, but are not limited to:

* Developing and implementing nursing home transitions program in collaboration with Medi-Cal Managed Care health plans and the IOA Inland implementation team

* Directing an interdisciplinary, multi-agency team of clinical experts in the Inland Empire who are responsible for transitioning and supporting individuals out of long term skilled nursing facilities into more independent settings

* Working in collaboration with Riverside and San Bernardino County providers to identify gaps in community-based services and supports for high-risk, low-income individuals

* Working in collaboration with Medi-Cal Managed Care health plans to identify gaps in long term services and supports for individuals enrolled in Medi-Cal Managed Care

* Collaborating with Vice President of Community Living Services and the Vice President of Strategic Partnerships, identify additional funding opportunities to support the expansion of services in the Inland Empire

* Providing final approval for all community living plans prior to discharge

* Developing internal and external budgets, forecasts, and variance analyses

* Partnering with IOA Human Resources to develop talent recruitment plans to ensure necessary staffing levels are maintained

REQUIRED QUALIFICATIONS

* Master's degree or higher in Administration, Public Health, Social Work, Nursing, Psychology, Counseling, Rehabilitation, Gerontology or Sociology from an accredited college or university

* Minimum 3 years supervisory experience in a medical or community-based setting

* Minimum 1-year experience as a Registered Nurse, Psychologist, or Licensed Clinical Social Worker in a medical or a community-based setting

* Experience in building and leading an efficient and effective multi-disciplinary team preferred.

* Broad understanding of community-based health and social services and pertinent policy issues

* Excellent interpersonal, verbal, and written communication skills

DESIRED QULAIFICATIONS

* Current/prior experience as a Licensed Clinical Social Worker (LCSW), Licensed Psychologist, or Registered Nurse

* Direct practice experience in behavioral health

* Demonstrated understanding of community-based resources and long-term care support and services

* Demonstrated understanding of behavioral health resources and behavioral health interventions

* Ability to develop and successfully manage large numbers of professional relationships

* Solid understanding of community-based case management

* Demonstrated ability to work collaboratively and to encourage collegial working relationships and team building within IOA and with outside partnerships

* Demonstrated ability to identify and develop new funding streams and partners

* Strong presence and creative thinking abilities
