Job Description

: SUMMARY: The Residential Registered Nurse coordinates medical services and facilitates mental health rehabilitation interventions and necessary supports to assist individuals with mental illness and substance abuse problems, if present, to achieve their rehabilitative, resilience and recovery goals following evidence-based practices. Promotes the physical and mental health of residents by applying the nursing processes. As necessary, will provide training to Residential Associates for certification within the facility for medication passes. Will provide training and documentation to maintain compliance with all state and federal requirements. Position works in secured and unsecured residential facilities.This is a non-exempt position which is subject to shift work, overtime and on-call.


1. Coordinates care to residents utilizing the nursing processes of assessment, planning, intervention, implementation and evaluation

2. Administers medication

3. Provides training and facility-level certification of proper protocols of medication passes and documentation to Residential Associates

4. Provides timely and accurate documentation.

5. Adheres to Oregon State Board of Nursing Standards and Scope of Practice for the Licensed Practical Nurse and Registered Nurse.

6. Effectively interacts with residents, significant others and other health team members. Ensures that health care information is distributed to appropriate staff, psychiatrist, and other medical personnel as necessary.

7. Develops and maintains therapeutic client relationships with close adherence to client-directed services.

8. Participates in the admission process of new residents to assigned facility.

9. Clarifies initial or hospital discharge medication orders and contacts the providers to review the residents status and medication plans.

10. Participates in the development and update of individual treatment plans in collaboration with the treatment team and clients.

11. Provides therapeutic interventions with residents in accordance with the treatment plan.

12. Implements treatment planning goals utilizing program-specific evidence-based practices.

13. Monitors safety and security of the treatment environment.

14. Assists or provides leadership in management of behavioral emergencies.

15. Assesses symptoms and manages immediate crisis including danger to self or others, and implements crisis intervention plan if necessary.

16. Coordinates care with medical providers, local foster care providers, senior and disabled services, and other service providers.

17. Provides life skills training appropriate to meeting client goals.

18. Assists with maintaining cleanliness and safety of facility.

19. Attends all mandatory meetings, participates in individual and group supervision. Maintains positive working relationships. Works as an active and cooperative member of treatment team.

20. Participates in chart reviews with supervisor and QA staff on a regular basis.

21. Works independently in the community, representing the agency in a professional manner at all times.

22. Transports clients to facilitate client-directed services.

23. Participates in orientation and training of new employees as requested.

Job Requirements

EDUCATION AND EXPERIENCE: Education and training sufficient to obtain Oregon Registered Nurse license.

Associates degree in Nursing. Experience in working in residential facilities and individuals with mental illness and/or substance abuse/additions.


Unrestricted, valid Oregon nursing license.

Possess business experience sufficient to meet the demands of the position, including proficiency with a variety of software applications.

Successfully pass criminal history check.

Provide proof of current, valid Oregon Driver License, comprehensive automobile insurance, and a safe driving record to use agency or personal vehicle for required duties.

Use active listening skills with clients, colleagues, and community partners.

Consistently present a positive, cheerful attitude, and professional dress and demeanor that reflect favorably upon the organizations image.

Handle frequent interruptions and changing priorities.

Use effective customer relations skills, including the ability to work with diverse people in a stressful environment, displaying an understanding of group dynamics and dealing with stress effectively.

Practice strong organizational skills and effective use of time.

PHYSICAL DEMANDS: While performing the duties of this job, the employee is frequently required to stand, sit, talk, and hear. The employee is regularly required to move about the facility and may be required to use stairs; use hands and fingers to operate, handle, or feel objects, tools, or controls; and reach with hands and arms. Requires sufficient mobility and balance to participate in ProACT (Professional Assault Crisis Training) training and to implement its principles.
