DUTIES AND RESPONSIBILITIES:


Responsible for primary coordination of all services delivered by article 16 clinic to the individuals we serve.


Prepare, complete, and submit treatment plans, addendums, and discharge plans


Process and monitor specific tracking grids to ensure that all information needed is complete and timely.


Occasionally coordinate appointments specific clinic services.


Obtain all the necessary consents required for admission to Art. 16 Clinic. Maintain charts and ensure all


information is contained in the medical records, i.e. consents, medical evaluations, rehabilitation evaluation.


Coordinate with Day Habilitation Staff/Treatment Coordination to facilitate services for clients.


Ensure that all appropriate parties receive notification and verification services, i.e. Care Coordinators, Medicaid Service Coordinators, parents, care givers, etc.


Provides all services in a legal and ethical manner


All other duties as requested and/or required by supervisors


QUALIFICATIONSTo perform this job successfully, an individual must be able to perform each essential duty (listed previously) satisfactorily. SUPERVISORY RESPONSIBILITIES - None


PRODUCTIVITY - Does not perform billable services. Is responsible for facilitating access to providers for individuals


EDUCATION and/or EXPERIENCE - Bachelors Degree


LANGUAGE SKILLS - Written and verbal fluency in English and ability to read and understand documents such as medical reports, assignments, etc. Ability to write routine reports and correspondence; ability to speak effectively with service providers and families. Bilingual in Spanish preferred.


MATHEMATICAL SKILLS - Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percentage and to draw and interpret bar graphs.


REASONING ABILITY - Ability to solve practical problems and to deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral or schedule form.


WORD PROCESSING - Must be able to use Microsoft Word, Excel, be comfortable with using different computer programs, with training.

While performing the duties of this job, the employee is regularly required to walk; to communicate verbally; to listen and hear via use of person to person and telephone contact. The employee must occasionally lift/move up to 10 pounds. Specific visual demands of this position include close vision (i.e. sitting and use of computer for extended periods of time, etc.).
